The Accommodation Request Form is used by students with disabilities or temporary impairments to request appropriate academic, physical, or support-related accommodations from the Disability Support Office (DSO) at Tishk International University. The purpose of this form is to ensure that students receive equal access to learning opportunities and university services through reasonable and individualized adjustments.
By completing this form, students help the DSO understand their specific needs and the barriers they may face in academic, campus, or residential environments. This information is used solely to develop an Individual Accommodation Plan (IAP) and coordinate necessary support with relevant university departments.
